2.  Information FIC Collects

FIC may collect "personally identifiable information" from you, such as your name, email address, phone number and date of birth. FIC will collect personally identifiable information from you when you voluntarily give FIC the information, such as when you enroll on the Site, complete a survey, or otherwise communicate with the Site.

The user name and password you create during registration is used to sign-in to the password-protected area of the Site and linked third-party websites.

Unless you tell it not to, FIC may use your personally identifiable information to contact you with information FIC thinks you may be interested in. If you do not want to receive this information, contact us. Email messages will also provide you with an opportunity to opt-out of receiving this information. Please note that changes to your preferences may not be effective immediately.

FIC may use third party contractors to help it operate its business and the Site or administer activities on its behalf, and may share your information with these contractors for those limited purposes.

You can review, change or delete your personal information by logging into the FIC account or by emailing/phoning customer service.

Protecting your information is important to FIC. FIC uses industry standard technology to protect the security of your information. Unfortunately, no data transmission over the Internet can be guaranteed to be 100% secure. As a result, while FIC is committed to protecting your information, FIC cannot ensure or warrant the security of any information you transmit to it. All of our security measures will be reviewed and considered complaint with HIPAA security and privacy regulations.
